New Clients
If you have never been to New Hartford Animal Hospital before,
we will ask for your name and phone number. We will also ask for
your pet's name, species and the reason your pet needs to be
seen.
What to expect …
For safety, we require all pets to
be leashed or in carriers. Upon your arrival, there will be some
forms you will need to fill out. Please arrive promptly, so that
we may process your information as quickly as possible. You can
download the forms here and fill them out before you arrive.
Bring copies of your pet's medical records, including a
vaccination history, pet insurance information and a sample of
your pet's stool. If you do not have copies of your pet's
medical record, you can request your previous veterinarian fax
them to us at 737-0136.
